An objective assessment on the level of care and general support required for optimal functioning.
A bio-psychosocial assessment is an overview of your care needs. The social worker will consider doctor and other allied assessments but will also do their own assessment of your basic and instrumental activities of daily living, your cognitive functioning, your emotional and spiritual needs, your social needs as well as the resources available to you.
We consider your illnesses, medication management, personal care management, finances, support systems, emotional needs and cognitive abilities when creating recommendations and safety plans. Personal care management includes your ability to mobilize, wash, eat, dress, communicate as well as your ability to shop, prepare food, do household tasks and manage your finances. They also consider your background and previous level of functioning.
You can then choose to contract us to assist you to implement these recommendations.

Assistance to find a suitable frail care. There are many factors to consider to ensure it’s the right fit for you, including your financial means, need for socialization or privacy, visual appearance and management style of the facility.
We have been to many care facilities and can assist you to find the correct one.
Completion of frail care social work assessment forms.
We have a 48 hour turn around time on requested assessments.

We assist to handle crises that come up unexpectedly.
We assist clients to process a new diagnosis, loss of independence, loss of a loved one or transition to a new environment.
Therapy requires commitment from the client. Many of our clients are not self-referred, so we’re experienced in offering gentle support to those not yet ready to fully commit. Our approach is warm, supportive, and non-judgmental, helping clients feel safe to begin when they’re ready.
We have social workers trained in palliative care, bereavement support and trauma debriefing. We also offer equine assisted counselling to clients.

Costs

In-patient consultations are covered by most medical aids, and claimed directly from the medical aid. Some medical aids might cover outpatient consultations and should be claimed by the member.
